APPOINTMENT CANCELLATION POLICIES
PLEASE READ CAREFULLY AND FOLLOW UP WITH STAFF FOR ANY QUESTIONS.
Here at ChiroCare Therapy, due to the high demand of patients seeking care, we find it necessary to charge for missed or no-show appointments and ask that you respectfully give us at least a 24 hour notice if you must cancel or reschedule. All appointment cancellations must be made over the phone to our office at (949) 888-0404. If we are unable to answer, you must leave a voicemail with your name and your appointment date & time. All No-Show Appointments will not receive a refund or be able to rebook with us unless full price is paid upon booking for the next appointment. Prepaid visits and packages expire 3 years from purchase date. No purchase limit. All prices subject to change.
PLEASE NOTE: We will do our best to remind you of your appointments (we will send you a text, email and app reminder if you’ve allowed us to contact you) but we ask that you make note of your appointment time(s) as well in case there happens to be any technical or software glitches on our end.
It is your responsibility to keep track of your appointments here in our office.
APPOINTMENTS WITH DR. STEVEN GENTRY
Our $49 New Patient Deposit is Non-Refundable as we reserve a 45 minute block with Dr. Gentry. If you need to cancel or reschedule a New Patient Appointment, please give us at least a 24 hour notice by calling our office. If it's within 24 hours, you will need to request another New Patient appointment with us and leave another $49 Non-Refundable deposit to reserve a new appointment time.
REPORT OF FINDINGS APPOINTMENTS
Our Report of Findings visit is for Dr. Gentry to go over any X-Rays with you & follow up on the assessment & treatment of your care, creating a care plan. We reserve a 30 minute block specifically for this and ask that if you need to cancel or reschedule an ROF Appointment, please give us at least a 24 hour notice by calling our office. If it's within 24 hours, you will need to request a new ROF appointment with us and leave another $40 Non-Refundable deposit to reserve another appointment time.
ALL OTHER APPOINTMENTS
All other Appointments with Dr. Gentry are reserved specifically for you so we ask that you please give us at least a 24 hour notice by calling our office. If it's within 24 hours, you will need to request a new appointment with us and leave a $25 deposit upon booking to reserve the new appointment time.
APPOINTMENTS MADE WITH OUR MASSAGE THERAPIST
Although our Massage Therapist often has a waiting list of clients, No-Show or Last-Minute Cancellations greatly impact their schedule. Recognizing that we do set aside the scheduled time just for you, have other clients to consider, and must maintain a smoothly running business, we now find it necessary to charge for No-Show Appointments and Last Minute Cancellations.
MASSAGE THERAPY APPOINTMENTS
Massages are Non-Refundable and May be Transferred. A Credit Card is Required on File or We Require a 50% Non-Refundable Deposit Upon Reserving your Appointment. If you need to cancel or reschedule, you must call our office at (949) 888-0404 at least 24 hours prior to your appointment or your Credit Card will Automatically be Charged Half of your Total Massage Cost and this charge will be Non-Refundable. If we cannot answer, leave your information on our voicemail. Cancellations made via text or email are not accepted.
CANCELLATION FEE: 50% of Total Massage Cost
*Appointments made within 24 hours are automatically subject to pay fees.
PREPAID APPOINTMENTS: Will be transferred to credit (minus No-Show fee) to be used later.
GIFT CERTIFICATES: Same policy applies.
LATE ARRIVALS: No fee is charged but your appointment will end at the scheduled time.
